We have designed our enrolment process to be as easy as possible and you can complete the entire process online. Our enrolment form will ask for your and your child’s Centrelink CRNs to claim fee assistance, details about your child’s health, immunisation, development, routines, parents or legal guardians, custody arrangements, and emergency contacts. All information you provide is strictly confidential.
Documents required
To enrol, you will need to provide copies of:
- Your child’s birth certificate
- Your child’s Immunisation History Statement from Medicare
- Your photo ID
- Any action plans for asthma or anaphylaxis
- Any Family Court orders
If you are unable to upload photos or scans of these documents, please bring the originals to the Centre and we can make copies. Changes to any details or documents must be made in writing so your records can be updated.
